What's Typically Covered by Insurance

All eye care at Mudgil Eye Associates is billed to your medical insurance. We do not bill vision-only plans. Coverage depends on your specific medical insurance plan.

Medical eye exams (covered by medical insurance and Medicare)

What is not covered by medical insurance

Some elective and refractive services are not billable to medical insurance and are out-of-pocket:

  • Premium IOL upgrades for cataract surgery (PanOptix, Vivity, Light Adjustable Lens, etc.)
  • Femtosecond laser-assisted cataract surgery (when used for refractive precision)
  • EVO ICL refractive surgery
  • Routine eyeglass prescriptions and contact lens fittings without a medical concern

Medicare and Cataract Surgery

Medicare Part B covers medically necessary cataract surgery, including the surgeon fee, the facility fee, the standard monofocal intraocular lens (IOL) implant, and one pair of standard eyeglasses or contact lenses after surgery. Medicare typically pays 80 percent of the Medicare-approved amount and you are responsible for your Part B deductible and 20 percent coinsurance, which is often paid by a secondary or supplemental plan.

What Medicare does not cover

  • Premium IOL upgrades like PanOptix, Vivity, the Light Adjustable Lens, Odyssey, and toric IOLs (out-of-pocket)
  • Femtosecond laser-assisted cataract surgery when used for refractive precision (out-of-pocket)
  • Routine vision exams when there is no medical concern
  • Eyeglasses or contacts for refractive correction outside of the one post-cataract pair

Patients who want to reduce their dependence on glasses after surgery can choose premium services including an advanced technology intraocular lens (IOL) and femtosecond laser-assisted surgery. The base cataract surgery is still covered by your medical insurance; the advanced technology component is the out-of-pocket portion.

Final pricing depends on the specific lens, whether you choose femtosecond laser, and any additional refractive correction. Dr. Mudgil's team provides a personalized written estimate at your consultation so you know exactly what to expect.

HSA and FSA Accounts

Most elective vision services at Mudgil Eye Associates are HSA-eligible and FSA-eligible expenses, including premium IOL upgrades, femtosecond laser cataract surgery, EVO ICL, and contact lens fittings. Many patients use HSA/FSA dollars to offset out-of-pocket costs for advanced cataract surgery and refractive procedures. Bring your HSA or FSA debit card on the day of payment, or submit a receipt for reimbursement.

We accept cash, personal checks, and major credit cards (Visa, MasterCard, Discover). Payment is collected at the time of service for any patient responsibility (copay, deductible, or out-of-pocket amount).

Why don't you accept vision insurance?

As a medical-surgical ophthalmology practice, we focus on diagnosing and treating eye disease and surgical conditions, all of which are billed to your medical insurance. If your visit is for a routine refractive exam without medical concerns, a vision-plan-participating optometrist may be a better fit; for any medical eye care we are happy to see you under your medical insurance.

How much does cataract surgery cost out-of-pocket?

For most patients with Medicare or commercial insurance, basic cataract surgery with a standard monofocal IOL is fully covered after your deductible and copay. If you choose a premium IOL upgrade (PanOptix, Vivity, Light Adjustable Lens, etc.) or femtosecond laser, those are out-of-pocket.
