If you have glaucoma, you've probably been told that lowering your eye pressure is the only proven way to slow the disease. For most patients, that means daily eye drops. But there's another option many patients don't hear about: SLT (selective laser trabeculoplasty), a quick in-office laser procedure that can lower eye pressure for months or years and sometimes replace drops entirely.

What SLT Does

SLT uses a specific wavelength of laser light to gently stimulate the eye's drainage system (the trabecular meshwork). The laser doesn't cut or burn, it triggers a biological response that improves the natural outflow of fluid from your eye, lowering pressure. The whole procedure takes about 5 to 10 minutes per eye in our office.

Who's a Good Candidate?

  • Patients with open-angle glaucoma
  • Patients with ocular hypertension (elevated pressure without nerve damage yet)
  • Patients struggling to remember daily drops
  • Patients experiencing side effects from glaucoma medications
  • Patients whose drops are no longer providing enough pressure reduction

Newly diagnosed glaucoma patients are also good candidates, recent studies show SLT as a first-line treatment can be as effective as starting with drops.

What the Procedure Feels Like

You'll come in for a quick visit. We'll put a numbing drop in your eye, place a special contact lens for the laser, and apply the laser pulses through it. Most patients feel nothing or a brief flicker of light. There's no pain. You'll be back at your normal day within an hour.

What to Expect Afterward

You may have some mild light sensitivity for a few hours. We'll put you on a brief course of anti-inflammatory drops for a day or two. Eye pressure typically starts to drop within the first 1 to 4 weeks and reaches its full effect by 1 to 3 months.

The pressure-lowering effect of SLT typically lasts 1 to 5 years. When the effect diminishes, the procedure can usually be repeated.

How Effective Is SLT?

For most patients, SLT lowers eye pressure by 20-30%, comparable to a single glaucoma medication. About 75-85% of treated patients respond meaningfully. Some patients reduce their drop burden, others come off drops entirely.

SLT vs Glaucoma Drops

SLT has several advantages over drops:

  • No daily routine to remember
  • No side effects like red eyes, eyelash growth, or stinging
  • No insurance battles for prescription refills
  • Effective even when patients can't reliably take drops (arthritis, dexterity issues, dementia)

The main downside: it's a one-time effect that wears off and may need to be repeated.

Insurance Coverage

SLT is generally covered by Medicare and most insurance plans for patients with glaucoma or ocular hypertension. We'll verify your coverage before scheduling.
