If you're considering cataract surgery and want to reduce your dependence on glasses afterward, you'll likely be choosing between three of the most popular premium IOLs: the PanOptix trifocal, the Vivity extended depth of focus, and the Light Adjustable Lens (LAL). Each has different strengths, and the right choice depends entirely on your lifestyle and visual priorities. Here's how they compare in plain language.

Quick Summary

  • PanOptix: Best if you want maximum glasses freedom for distance, intermediate, and reading.
  • Vivity: Best if quality nighttime vision matters more than full reading independence.
  • Light Adjustable Lens: Best if you've had previous LASIK, have unusual corneal shape, or want the most precise refractive outcome.

PanOptix: Trifocal Glasses Freedom

PanOptix is a trifocal IOL, meaning it provides three distinct focal points: distance, intermediate (computer arm's length), and near (reading). The vast majority of PanOptix patients are able to read a menu, work on their phone, drive, and watch TV without glasses.

The trade-off: the optics that create three focal points also create some halos and starbursts around lights, especially at night. For most patients these diminish significantly within the first few months, but if you do a lot of nighttime driving, this is worth knowing about.

Best for: Active patients, frequent phone and tablet users, anyone who wants maximum independence from glasses and is willing to tolerate some nighttime visual phenomena.

Vivity: Extended Depth of Focus with Clean Nighttime Vision

Vivity is an extended depth of focus (EDOF) lens. It provides excellent distance and intermediate vision with very minimal halos or glare. It does not provide full reading vision; most Vivity patients need reading glasses for fine print like medication labels or restaurant menus in dim lighting.

What makes Vivity unique is its quality of vision. Patients consistently describe their vision as feeling natural with very few of the visual artifacts that can come with multifocal lenses.

Best for: Frequent night drivers, screen-heavy lifestyles, patients who prioritize image quality and are comfortable using readers occasionally.

The Light Adjustable Lens: Fine-Tuned After Surgery

The Light Adjustable Lens (LAL) by RxSight is the only IOL whose power can be adjusted after it's implanted. After surgery and a brief healing period, you'll come into the office for a series of light treatments. During each treatment, UV light reshapes the lens to fine-tune your vision.

This is particularly powerful for patients who have had previous LASIK, RK, or other refractive surgery, where calculating the right IOL power can be challenging. It's also a great choice for perfectionists who want the most precise possible outcome.

The trade-off: you'll need to wear UV-protective glasses outdoors and during waking hours for about 4-6 weeks while we complete the adjustment treatments. Most patients find this trade-off well worth it.

Best for: Post-LASIK patients, those with unusual corneal shape, perfectionists, anyone who wants the option to fine-tune their vision after surgery.

What About Cost?

All three are premium upgrades over the standard monofocal lens that Medicare covers. Out-of-pocket cost for premium IOLs typically ranges from $800 to $4,500 per eye depending on the technology and any additional procedures. We provide every patient with a written estimate before scheduling. See our cataract surgery cost page for more details.

How to Decide

The honest truth: the best IOL is the one that fits your specific eyes and lifestyle. During your cataract evaluation we measure your eyes, ask detailed questions about your typical day, and walk through your options with no pressure. Some patients are excellent candidates for PanOptix; others should never get a multifocal lens. We'll tell you what we'd recommend for your specific case.
