When you are preparing for cataract surgery, you will hear about two approaches: traditional (manual) cataract surgery and laser-assisted cataract surgery. Both remove your cloudy lens and replace it with a clear intraocular lens, and both have excellent, well-proven outcomes. The difference is in how a few of the steps are performed.

How traditional cataract surgery works

In standard cataract surgery, the surgeon uses fine handheld instruments to make a tiny incision, open the front of the lens capsule, and break up the cataract with ultrasound (a technique called phacoemulsification) so it can be gently removed. The new lens is then placed. It is a quick, comfortable, highly refined procedure that surgeons have performed successfully for decades.

How laser-assisted cataract surgery works

Laser-assisted surgery uses a femtosecond laser to perform several of those same steps with computer guidance. Working from a detailed image of your eye, the laser can:

  • Create the corneal incisions
  • Open the front of the lens capsule in a precise circle
  • Soften and segment the cataract before removal
  • Make fine corneal adjustments to reduce astigmatism

The rest of the procedure is the same, and the surgeon remains in control throughout. The laser simply adds a layer of precision to those specific steps.

Is one safer or better than the other?

Both approaches are very safe and deliver excellent vision for the vast majority of patients. Research has not shown one to be universally superior. Instead, the laser tends to add the most value in specific situations, such as correcting astigmatism or placing a premium lens, where precision has a bigger payoff. For a straightforward case with a standard lens, traditional surgery is an outstanding choice and is covered by insurance.
