What's Wrong With the Standard Drop Regimen?

Standard post-operative drops typically include an antibiotic, an anti-inflammatory NSAID, and a steroid. The reason is sound: each plays a distinct role in preventing infection and managing healing inflammation. The problem is human compliance. Studies show that even the most diligent patients miss roughly 30% of their drops. Patients with arthritis, vision problems, dementia, or simply busy schedules miss more.

And drops are expensive. Even with insurance, three different prescription bottles can cost a couple hundred dollars in copays.

How Dropless Cataract Surgery Works

At the time of cataract surgery, instead of prescribing post-operative drops, we inject a compounded medication directly inside the eye. This single injection contains both the antibiotic and the steroid, and it releases slowly over several weeks. The medicine is delivered exactly where it's needed, in the precise dose needed, with no compliance issues.

The procedure itself is identical to standard cataract surgery. The only difference is the addition of the intraocular injection at the end. Recovery is the same or smoother because the medication is consistent rather than drop-dependent.

Who's a Candidate?

Most cataract surgery patients are candidates for dropless surgery. We typically don't do dropless on patients with certain pre-existing eye conditions like uveitis or those who would benefit from a more individualized drop regimen. We discuss it as part of every cataract surgery consultation.

Dropless is especially useful for:

  • Patients with dexterity limitations from arthritis or stroke
  • Patients with memory or cognitive issues
  • Patients on multiple medications already
  • Patients who simply prefer the simplicity
  • Caregivers managing eye care for an elderly parent or spouse

Is It Safe?

Dropless cataract surgery has been studied extensively and has a strong safety record. The intraocular injection has been used by ophthalmologists for years. We've performed dropless surgery on hundreds of patients with excellent outcomes.

Cost and Coverage

Dropless is covered as part of cataract surgery, with the medication itself costing approximately $50-$100 depending on insurance. This is typically less than what patients pay in copays for the three different drop bottles. Many patients find this surprising.

Why Aren't More Practices Doing This?

Dropless cataract surgery requires using a compounding pharmacy partner to prepare the intraocular medication. It's also a slightly different surgical technique. Many practices simply haven't adopted the protocol. We've been doing dropless cataract surgery longer than any other practice in Chester County and have refined the process to a point where we offer it as the standard option for cataract surgery, not a special upgrade.
