New Patient Forms In order to facilitate your first visit to Mudgil Eye Associates and minimize your wait time, please download the two forms below, and bring the completed forms on the day of your visit. Please also bring your insurance cards. We accept checks, cash and credit cards for co-payments. **Patients who have an HMO insurance (such as Keystone Health Plan East, Aetna HMO, or Tricare Prime) MUST have a referral from their primary care doctor at our office prior to the time of the visit, or the visit may have to be rescheduled.** Please make cancellations at least 24 hours prior to the scheduled appointment; failure to do so may result in late cancellation fees. Deductible,coinsurance, and/or copay are due at the time of the service. Thank you
